Required Forms for Participation in All Sports.
In addition to a physical, there are several forms that must be filled out prior to any participation in a high school sport.  You may download them below:

• Sports Physical Screening Form (mandatory for physicals.  Physicals are good for one year.  This form was updated in May 2011.
• Sports Medical Health Insurance Form   All athletes must have either basic health insurance or purchase health insurance through the district.   This form was updated in April 2012. 
• Physical Exam Form
• Pre-History Medical Form
• Emergency Card
